How to Apply to The SocialCareRx Initiative

Frequently Asked Questions: Applicants

.

We are only able to process applications every 14 to 21 days. Please see local resources for emergency funding.


  1. You have been diagnosed with opioid use disorder (opioid addiction), have been actively participating in outpatient treatment for at least 6 months and give consent for us to verify this
  2. You live in one of our designated jurisdictions
  3. Your household meets our income criteria (our criteria are based on a combination of the area median income where you live and the federal poverty level) 
  4. You are at least 18 years of age
  5. Your household has at least one active bank account OR has filed a tax return within the past two years
  6. You have access to a computer and the internet
  7. If you are housed and rent, your household has a legal lease that your landlord and a member of your household both have signed. This lease must show the monthly cost of your rental, whether or not you pay all of this amount yourself



Every situation differs but all applications require:


  1. A form of a picture ID (including non-traditional forms of ID like a booking or arrest photograph if traditional ID's are not available)
  2. Income verification 
  3. Bill or obligation (reason for cash assistance request)  
  4. An explanation of how having this particular bill/obligation paid can contribute to your social or economic stability in the area of shelter, housing or employment
  5. Evidence of current treatment for opioid use disorder


Other supporting documents may be accepted or requested.


Given the nature of the assistance, supporting documentation must always be submitted.


We pay the funds directly to the vendor or creditor after verification of the documentation you provide.


Our cash assistance amounts range from $250 to $750.

There are no limits to the number of applications you may place before you are funded. Once your request is funded you will then have to wait 12 months before you can apply again to allow others to receive support from this small funding resource.


Once we receive documentation  it is our goal to process documentation as fast as possible. In times of high volume it may take a few additional days before we view your documents. We are only able to offer each application one round of review, so please make sure your documents are accurate and current!
